Out-patient medication administration has been identified asthe most error prone procedure amidst the entire medicationprocess. Most of these errors were made when patientsbought different prescribed and over-the-counter (OTC) medicines from several drug stores and use them at homewithout little or no guidance. Most of times patients may forget to take the medicines at proper time as per the specified in the prescription which may cause in late recovery from the disease/illness. So it is necessary to take proper medicines in proper quantity at proper time. Most medication administration errors were made when patients bought prescribed and over-the-counter medicines from several drug stores and use them at home without little or no guidance. Elderly or chronically ill patients are particularly susceptible to these mistakes. In this project, we introduce Wedjat, a smart phone application designed to help patients avoiding these mistakes.
